- When are you planning on arriving and departing?
- Which additional events are you planning on attending? (Friday Night Social, Spouse Luncheon, Auxiliary Luncheon, Company Grade Luncheon, Banquet)
- Would you like to pay your membership (Annual or Lifetime) dues at this time?
- Will you be bringing children that require day care or that will be participating in youth activities?
- Credit Card information for preferred on-line payment.
For Exhibitors:
- Names and contact information for both primary POC and those staffing booth(s).
- Sponsorship only or is booth space requested?
- Number of booth spaces needed.
- Payment information.
